Why was my COI rejected?

A Certificate of Insurance (COI) may be rejected for a number of reasons.  Appruv will notify you of any deficiencies and provide a sample with instructions to meet your clients insurance requirements.  The most common reasons for a COI to be rejected include: 


  • The company name or address in the INSURED box does not match the information in your Appruv account
  • A policy number is not listed for one or more of your insurance types
  • A policy effective date or expiration date is not listed for one or more of your insurance types
  • The policy effective dates are not current (expired)
  • The policy limits do not meet your clients requirements
  • The required endorsements are not provided for Additional Insured or Waiver of Subrogation
  • The CERTIFICATE HOLDER name is not correct


For detailed instructions regarding your clients insurance requirements please review the sample COI provided on introduction page of your questionnaire.  To view this information login to Appruv and find your client logo on your dashboard then select Complete Missing Items (or View Prequalification if you have already submitted your application).  On the left sidebar select Introduction and you will see a link to the Sample COI as well as any other documentation and forms provided by your client.
