What if I cannot provide the past 3 years OSHA 300A?

It is rare but in some cases a company may not be able to provide the previous 3 years completed OSHA 300A.  For example a new company that may have just formed will have no historical data for safety performance.  In that case please provide an explanation along with the date of establishment, on company letterhead signed by an authorized representative of the company and upload it to Appruv in place of your OSHA 300A forms.  An Appruv Support Specialist will contact you if any additional information or documentation is needed from your company.
