OSHA 300A Overview

The Summary of Work-Related Injuries and Illnesses (otherwise known as OSHA Form 300A) must be submitted each year in order to verify the safety metrics submitted in your application.  OSHA Requires that all companies with 11 or more employees for the previous calendar year keep logs to track work related injuries and illnesses.  


Requirements for submitting your OSHA 300A to Appruv include: 

  • The effective year
  • Establishment Name and Information (must match your Appruv account)
  • Annual average number of employees
  • Total hours worked by all employees for effective year
  • Totals for columns G - L
  • Signed and dated by an officer of the company


Note: For privacy purposes please remove or redact all employee information before submitting your OSHA forms.
