How do I complete the Annual Summary (300A)?

To complete the annual summary you must total the columns on the OSHA 300 Log and enter the calendar year covered and the total hours worked by all employees for the year.  Remember if you had no recordable cases for the year you must still complete the log and enter zeros for each column total.


A company executive must certify that he or she has examined the OSHA 300 Log and that he or she reasonably believes that the annual summary is correct and complete with their signature, title and date on the 300A Summary.


Who is considered a company executive? 


A company executive is considered to be:

  • An owner of the company;
  • An officer of the corporation;
  • The highest ranking company official working at the establishment; or
  • The immediate supervisor of the highest ranking company official working at the establishment.
